This form is prepared by the Offender Records Section to identify the contents of boxes of records sent to the State Archives for permanent storage. Forms include the following information: year sent to Archives Center, transmittal number, series number as shown on records retention schedule, schedule number, box number, description of files as indicated on schedule, agency, and division sending records to Archives Center, and name and institution number of each inmate whose case file is included, or, if an inmate has been sentenced more than once, release number.
